ICAN Outpatient Treatment

The outpatient ICAN Program is part of the Day Hospital at Sagamore Children's Center.

  • serves children K- 8th grade.
  • collaboration between Western Suffolk BOCES and the division of Sagamore Outpatient Community Services.
  • Admission by referral from school district
  • A minimum Full-Scale IQ of 55 is required

Students receive daily family communication, a flexible, individualized care plan and feedback to school districts.

Program Hours:
8:45am - 4:45pm Mon- Fri.
K- 3rd grade program ends at 3:45pm.
School may be closed on or during certain holiday periods –the therapeutic and recreational services will remain open and run daily from 10:00 am - 3:00 pm.

Intensive Day Treatment – 30 Day

The Day Hospital is a unique outpatient program for young people with serious emotional and behavioral difficulties. Children and adolescents at risk for hospitalization or those who need an extra step between hospitalization and community living are good candidates for the Day Hospital.

It offers many of the benefits of inpatient care in a community-based environment. Participants receive the services of a full mental health team as well as an academic curriculum during their stay.

The Day Hospital is located on the Sagamore campus.
School hours: Monday – Friday 8:45am-4:45pm.

Children’s Mobile Integration Team (C-MIT)

C-MIT is a voluntary, home-based program offering support for children and their families. MIT aims to minimize the child’s need for ER, CPEP and inpatient hospitalization.

C-MIT uses a three-prong approach to working with youth and their families:

Social Workers:

  • assist with case management
  • determine appropriate mental health interventions.

Family Peer Advocates:

  • offer case-specific and general parenting instruction
  • support when interacting with school districts.
  • familiar with Special Education services
  • Able to accompany a parent/guardian to CSE.

Mental Health Therapy Aides (MHTA):

Patchogue
  • assist in helping children practice and reinforce therapeutic skills,
  • offer respite both in and out of the home, and
  • role model strengths-based adult-child interactions.

Collaborative staffing that includes both teachers and mental health professionals ensures an effective educational/therapeutic experience. This is ideal for young people who need a smaller setting to help them manage their emotional and behavioral disturbances.
